EDIT: If your student does not bring a packet home- they can be printed from Physical Packet under the Document section. David A. Milano, FNP from Heartland Integrative Medicine will be at Field School on Wednesday, May 14th to complete physicals for students. If all students are not completed on May 14 then staff from Heartland Integrative Medicine will return on Tuesday, May 20th. Students must have an up-to-date sports physical on file at school before being allowed to participate in any sports tryout, practice, or event. This physical would cover your student for the entire next school year. If you would like your child to receive a physical on May 14 please complete the consent form and complete side 1 of the Pre-participation Examination form. The back side will be completed by Heartland Integrative Medicine staff on the day of the physical. The cost of the physical is $20 and cash or check will be accepted. Packets going home soon!
